For your initial office visit, wear comfortable and condition appropriate clothing.
In the event of a cancellation, and as a courtesy to other patients waiting for an appointment time, kindly call our office at least 24 hours in advance. We reserve the right to charge for last minute cancellations and no-shows.
Most insurance companies require a referral from an attending physician. Please bring your referral slip with you to your first scheduled appointment.
We are a provider for most local and major insurance companies. We will be happy to assist you by verifying your benefits or you may contact your carrier prior to your first visit. For additional insurance or billing inquiries, please call our administrative office.
